The Yizkor Memorial Prayers are printed together with the Book of Remembrance in one volume. Donors, as well as their departed loved ones, are listed in the booklet. The booklet will be used on Yom Kippur, Shemini Atzeret, the last day of Passover, and the second day of Shavuot when Yizkor is recited. To make the connection to these moments of remembering loved ones more intimate and meaningful, please complete the form below and the names of your loved ones will be included. The deadline for your submission is August 20th. Please contact us if you have any questions regarding your previous submissions.
